AYSO Matrix meeting - May 10, 2010
May 15, 2010 - DEADLINE!!!! - olders (U15 and up) registrations are due to the registrar no later than May 15th. The following are needed for registration:
- 1 copy of the player registration form with a wet signature on it and IF the player is new to Matrix, I will need an copy of the birth certificate.
- An Administrator Assignment form for each administrator for each team they are to be assigned to
- the Team Application form complete with a roster and any seeding info which will be needed at the seeding committee
Please Be sure that all forms have the Team # on them. If you are a returning team the number stays the same with one exception, the age advances one year ie. if last year you were 0131-02GC14-0279 this year your team # will be 0131-02GC15-0279.
The League is AYSO MATRIX and the club is your region i.e. La Mesa, San Marcos, etc.
Please be aware that all administrators must have cleared risk management and all coaches must have the CYSA coaching license.
