Useful information for Team Managers
Please contact your Regional Representative for proper procedures before completing and turning in Team Paperwork. Team paperwork deadlines are the responsibility of the Regional Representative.
Please use the Matrix Team Application form to register your team and apply for seeding. Please download and send the completed form to janumatrix@yahoo.com. You may include it with your Team Information paperwork.
You may fill in player registration forms and adminstrator assignment forms online, then print them to turn in with your Team Information paperwork to the Matrix CYSA Registrar. Parents, Coaches or Managers should print 2 copies of each form and retain one original for your team binder. Player registration forms for all players new to Matrix and/or CYSA should be accompanied by a copy of the player's birth certificate. CYSA FormsDon't forget to have your parents register their children with AYSO on-line.
Parents please check with your region before registering your child.
